A pharmacy runs on one wholesaler, on a contract. That wholesaler brings about 90% of what it sells, every day. When the pharmacist says “we don’t have it,” the medicine is not gone. It is on a shelf in another warehouse, three states over, at one of the thousands of smaller wholesalers whose whole business is having what the big truck doesn’t.

The pharmacist doesn’t know which warehouse. The warehouse doesn’t know he’s asking. “We don’t have it” is a missing phone call. That is the backorder crack, and it is the entire market.
